Smart Benefits: IRS Publishes Final ACA Forms for 2017

The IRS recently released forms and instructions for Affordable Care Act reporting for 2017.

B Forms: These forms are filed by providers of minimum essential coverage to report information about individuals to whom coverage was provided. There were no substantive changes to either 1094-B, Transmittal of Health Coverage Information Returns, or 1095-B, Health Coverage, for the 2017 tax reporting year.

C Forms: These forms are used by Applicable Large Employers to provide the IRS the information it needs about offers of health coverage and enrollment in health coverage for their employees.

  • 1094-C, Transmittal of Employer-Provided Health Insurance Offer and Coverage Information Returns: This form is used to report to the IRS summary information for each ALE member and to transmit Forms 1095-C to the IRS. The only change on the 2017 version from the 2016 form is the change in box C of line 22 from “Section 4980H Transition Relief” to “Reserved”. That’s because all forms of transition relief that were available to some employers have now expired.
  • 1095-C, Employer-Provided Health Insurance Offer and Coverage: The form is used to report information about each employee to the IRS and to the employee. ALEs that offer employer-sponsored self-insured coverage also use the form to report information to the IRS and to employees about individuals who have minimum essential coverage under the plan and aren’t liability for the shared responsibility payment for the months of coverage. For the 2017 reporting year, there are no changes to this form.
