Health Professional in RI Reprimanded for “Forging Signature & Abandoning Patent”

A health professional has been reprimanded by the Rhode Island Department of Health (RIDOH) for reportedly forging the signature of a patient - and leaving her shift early.

RIDOH sanctioned licensed nursing assistant Juliana Carrion Brito for “unprofessional conduct” under Rhode Island General Law.

About Case 

According to RIDOH, Brito was employed as a nursing assistant by a staffing agency in November 2024 and assigned to work at the home of a patient “with a shift ending at 5:30 PM.”

“On December 13, 2024, the patient called the staffing agency and reported that [Brito] had been leaving her home at 4 PM and not 5:30 PM as scheduled, thus abandoning the patient in need of her care,” wrote RIDOH. 

“The patient also reported that she, the patient, did not sign [Brito’s] electronic timecard when [Brito] left her home at 4 PM. Further investigation demonstrated that [Brito] had forged the patient’s signature on the electronic timecard,” RIDOH continued. 

RIDOH said the conduct “constitutes unprofessional conduct” and issued the reprimand on March 7, 2025.
