The Dirty 18 — RI Nursing Homes Fined More Than $100K by Feds

We have unveiled alleged sexual assaults, failures by the Rhode Island Department of Health (RIDOH) to track and disclose violations by staffers, and inconsistent rankings by national publications.

For families seeking for guidance, the conflicting data is difficult to manage and find.
GoLocal has reviewed federal data and identified the 18 nursing homes that have been fined more than $100,000 in the past four years.
Fines are levied by the Centers for Medicare and Medicaid Services (CMS). Nursing homes with minor deficiencies do not get fined.
One nursing home has been fined significantly more than all others. Elmhurst Rehabilitation & Healthcare Center, located at 50 Maude Street in Providence, has recorded 86 total deficiencies, of which five were identified to be serious deficiencies, and the home has been hit with $622,463 total fines.
RED FLAGGED
Six nursing homes have been “Red Flagged” — these are homes that the feds have deemed as in need of “Special Focus.”
Five of the six have been fined $100,000 or more. And, one — Trinity Health And Rehabilitation Center — has been fined $97,362 for 50 deficiencies and five of those were deemed serious. Based on this record it is unclear why Trinity has not been fined more.
