Providence’s HopeHealth Receives National Award for Home Care Excellence

Providence-based HopeHealth Visiting Nurse has been awarded Home Health Quality Improvement’s (HHQI) Cardio Milestone Five for their patient’s overall cardiovascular health improvement and dedication to home healthcare excellence. 

“Each year, HopeHealth Visiting Nurse strives to improve and deliver the highest quality of care possible to our patients. We’re honored to have been recognized for our continued improvement, and we look forward to another year of providing excellent care,” said Diana Franchitto, president & CEO of HopeHealth.

HHQI Milestone Five is the highest achievement level of HHQI.

HopeHealth is the first agency in Rhode Island to achieve the honor.

“Becoming the first agency in the state to receive this important milestone speaks to our dedication to our patients. We want to thank the Home Health Quality Improvement campaign for their dedication to the cause as well, and for recognizing our hard work,” added Franchitto.

About HHQI

HHQI is a national campaign to help home health providers reduce avoidable hospitalizations and improve care quality.

Cardio Milestone Five is designed to assist home health agencies in demonstrating sustainability of cardiovascular quality measures.

Currently, more than 2,400 home health agencies are participating in HHQI’s Cardio Milestone program.

Of those, only five other agencies have achieved Milestone Five.

HopeHealth Visiting Nurse was also recently recognized for achieving optimum care for patients with serious illnesses, becoming the first agency in the state to be certified in community-based palliative care by The Joint Commision.
